Hidden items
I always found it so cool in SAO when Kirito would tell about some secret item he heard about somewhere. I feel like so items should make players explore usually avoid scenery in order to find quest items/rare items.

On the idea, I like hidden content.
The "problem" is always that hidden content is actually hidden for no less than a couple of weeks. Once someone discover something and talk about it, the information is on all wiki/forums so it's not hidden anymore.
Worse, if there's a lot of hidden content, people will feel forced to checks wikis and online guides. It removes a lot of the adventure and discovery part. This is why I think that if we create any type of hidden content, we have to make it change constantly. Like if it's an item, it's never on the same place. Doable, but a lot of work if we have a lot of hidden content.
